Health care transition tools lacking in pediatric and adult liver transplant centers

Collaboration is needed between pediatric and adult liver transplant centers to aid in the improvement of the health care transition (HCT) process due to the increased risk of medical complications and morbidity after transferring from pediatric to adult services, according to a poster presented at The Liver Meeting 2021.

Researchers surveyed members of the Society for Pediatric Liver Transplantation (SPLIT) and all active United States adult liver transplant programs to determine what HCT approaches are currently being used.

The survey found that:

-Pediatric centers focus on HCT with “formalized tools” more so than adult centers.

-Providers caring for adults assessed psychosocial health, privacy/consent, and insurance less often.

-Both pediatric and adult providers had similar thoughts on perceived barriers and provider identified important outcomes measures
