Early proteinuria levels predict complete renal response in lupus nephritis

Several factors taken at baseline and 3 months after starting induction treatment can predict complete renal response (CRR) and renal flares once CRR is achieved, according to a study.

In this retrospective cohort study, 104 patients with proliferative LN (class III/IV) were followed for 36 months. CRR was defined as s proteinuria <0.5g/day with normal renal function and renal flare was defined as proteinuria >1g/day after CRR attainment.

In total, 91.7% of participants reached CRR, within a median time of 6 months. Independent predictors of CRR included Proteinuria <2g/day at baseline and 3 months after the start of induction therapy. Of the patients who reached CCR, 18.4% had renal flares after a mean time of 16.5±8.6 months. Independent predictors of renal flares included age ≤25 at LN diagnosis and positive anti-RNP.
